2.9 Closing Video

[Back to Shop Floor – John and Liza]

Closing Message

John (quietly): “They weren’t even supposed to do it… It was supposed to be finished the next day.”
Liza - Ayaan and Neil didn’t ignore safety—they misjudged the risk.
Ayaan estimated the job very small with almost no serious risk, this was due to his low perception towards risk. Instead of seeing a high-voltage, high-current panel, he saw a small job he just needed to finish. Neil followed, trusting their experience. If Ayaan had good risk perception he would have not done the job. Due to his low risk perception he choose to do the job. The perception was so low they dint wear the PPE which was available next to the MCCB panel. And because of that skewed perception, two lives were lost.

The lesson: - Risk isn’t just about the task—it’s about your mindset when approaching it. If you have a low risk perception your actions may get influenced adversely.
